Where can I download release forms?

Answer

Download the photo/video/audio recording release forms using the links below. Generally, these forms are used to record people who are not UC faculty, staff, students or affiliated with UC-sponsored programs like 4-H and Master Gardener.

Both are Acrobat PDF versions. Fill in event and date fields in Acrobat before you print, or by hand after you print. Revised 01/2009.
